• Intake Form - This form is helpful to bring to each appointment your pet has.  It will give the vet a clear picture of the reason for your visit as well as any medication you may be giving and the food you are feeding them.

• Eastern Medicine Exam - These forms are mandatory for all clients whose pets have a consult with Dr. McComas regarding Acupuncture and Eastern Medicine.
